One of my employee has applied the e-visa online application form and we have got the approval letter also. But there is no exit and entry form attached with the approval letter. So it is mandatory that exit and entry form is required while entering the airport.

Dear RASHMI,
Normally, when we send you approval letter, we also give you the Entry and Exit Form in the email. However, if you did not receive this form, you can take it at Visa Landing Counter to fill and get visa stamp there
