Form to fill in

Hello
Just a question in the above 4 steps it says in step 3-
Get visa approval letter via email (attached with entry-and-exit form)
Print out the letter and the entry-and-exit form
But in my email with the letter had the link to application for Vietnamese visa. Do i have to fill this form out as well or is it meant to be the link to entry and exit form.
