For the first part of your question, let's consider this example for a clearer understanding:

You have registered to enter Vietnam on July 27 and leave on August 20

_ You can arrive in Vietnam on or any day after July 27 as long as the date of entry is within the visa validity. However, you CANNOT enter Vietnam BEFORE July 27.

_ You can leave Vietnam before or on August 20 but you MUST NOT get out of Vietnam AFTER August 20. 

Secondly, only the natitonalities on the list of Visa Exemption policy of Vietnam Immigration Department do not need a visa for a specific number of days and as a result do not have to pay for stamping fee.

Unfortunately, USA is not on the list so you need a visa to travel to Vietnam. You can apply online on our website to get visa on arrival. The process is quite fast and easy. You only need to fill in the form then make online payment. After 2 working days, we will send you the approval letter via email.
