Dear Steve,

For USA passport holders, you need to apply for visa on arrival. Based on your itinerary, one month entry visa is the best choice for you. For the fee, you can refer at this link https://www.vietnam-evisa.org/visa-fee/united-states-of-america.html 

For Thailand passport holders, no need viet nam visa. She is exempted for visa in 30 days. However, please remember that the passport must be valid at least 6 months from the entry date and  the previous visit must be far away from the next one at least 30 days
