What is “entry and exit form”?

Entry and exit form is the important document that you have to lodge to Vietnam Immigration Department when entering Vietnam. You have this form easily with 2 below ways:

A – At the airport gate, you can get Entry and exit form in “Landing visa” counter. It will be handed out from officer in order for you to fill your information. However it takes you about 7-8 minutes to fill up.

B - If you enter Vietnam with VOA (Visa on Arrival), the agent who process visa will send to you an approval letter and a link to download entry and exit form via mail. By this way you can fulfill your information in the form at home, stick your photo and bring it to the airport to get your visa.

You are required to enter the exact personal information of the applicants. Your application will be returned to you if you do not accurately finish this form. This is the form and it requires your “entry and exit” information in Vietnam.

The instructionto fill in the “Entry & Exit Form” including 14 parts:

  From number 1 to number 5 are some personal information of passenger such as:

No1. Name and surname: fill your full name in block letters as in your passport

No2.Date of birth: fill clearly in dd/mm/yyyy order

No3. Sex: male or female

No4. Place of birth: fulfillcity and country name

No5. Nationality: include in nationality at birth and current nationality

Number 6 is your passport details:

-      Passport number: write your exact passport number (both number and alphabet letter)

-      Date of issue and expiry date

-      Issue by: you can check your passport for details.

-      Kind of passport: there are 4 types of passport “ regular passport, official passport, diplomatic passport and passport card ( noted: passport card is the same basic as a regular passport)

Number 7 concerns to your profession, place of employment and telephone number:  This information must match the records as you applying for a visa.

Number 8 is present address and telephone number: fill in your address in the country that you are coming from

Number 9 is children accompanying: you must fill children’s full name, date of birth and relation.

Number 10: purpose of entry and exit: fill in your purpose (vacation, business, study, visit….)/Name, address of office, organizationor full name, relation, address of your relatives to contact or visit while staying in Vietnam.

Number 11: propossed duration to entry and exit Vietnam: information has to match the visa

Number 12: entry and exit times

“single” for enter and exit Vietnam one time in the visa valid period

“multiple” for enter and exit Vietnam more than one time in the valid period noted in your visa

Number 13: port of arrival and departure: NoiBai airport, Ho Chi Minh airport or Da Nang airport

Number 14: your confirmation“ I swear the above declarations are true for which I shall take responsibility”

Done at ……on ….: fill the name of the airport where you arrive and date when you are entering Vietnam

Note: The photo size is preferred to be 4x6 cm. However your photo size is smaller than the standard size that is accepted.

 The photo must be the latest one or as your passport photos. If you don’t have photo you can take photo in the Vietnam airport with  the fee 5USD.
